Before attempting to add a new faculty member please make sure they are not already present in the database! This form is only to add teachers that do not already exist in the database. Please do not enter TA's / graduate assistants that most likely will only teach/assist one or two terms of one class.

Please be aware that the data you enter here does not go directly into the database, the professor you submit will be reviewed first before going into the database!
